Health Requirements & Forms

New York State Public Health Law and Alfred State Policy require students to submit certain health forms and immunization records before arriving on campus.

Key Requirements

  • Immunization Proof (MMR): All full-time students (6+ credits) born in 1957 or later must show proof of immunity to measles, mumps, and rubella (usually 2 doses or blood test results).
  • Meningitis Requirement: Students must show proof of a meningococcal vaccine within the last 5 years or electronically decline the vaccine after reviewing the information.
  • Health Forms: Complete a Health History form and a Communication Preferences form via the Student Health Portal. (Students under 18 also need a “Permission to Treat” form signed by a parent or guardian.)

Available Forms

The following forms are available via the Student Health Portal. Click the link, log in, and go to "My Forms" for access.

Meningococcal Disease

Meningococcal disease is caused by the bacteria Neisseria meningitidis. It can cause serious infections in your blood and in the membranes covering your brain and spinal cord (meningitis). The disease develops quickly and can be deadly.

Who is at Risk?

  • Teenagers and young adults
  • Infants under one year old
  • People living in crowded settings like dorms or military barracks
  • Travelers to areas like the “meningitis belt” in Africa
  • Individuals with a damaged or missing spleen, or with sickle cell disease
  • Those taking Soliris® or with an inherited immune disorder
  • People exposed during an outbreak
  • Laboratory workers handling the bacteria

Symptoms:

  • A high fever
  • Headache
  • A stiff neck
  • Nausea and vomiting
  • A red or purple skin rash
  • Weakness and severe illness
  • Sensitivity to light

How It Spreads:

The disease spreads through droplets when an infected person coughs, sneezes, or talks. Many carriers show no symptoms.

Treatment and Complications:

Early antibiotic treatment is crucial. Without prompt treatment, up to 15% of cases may be fatal, and survivors may experience long-term complications such as hearing loss, brain damage, kidney damage, or limb loss.

Prevention:

Vaccination is the best prevention. Vaccines are available for anyone 6 weeks and older. For teenagers, two doses of the MenACWY vaccine are recommended (first at 11-12 years and a booster at 16 years). Teens may also get the MenB vaccine per their doctor's advice.
