Daily Screening Attestation App

We are so excited to have students in person and on campus! If you would like to monitor your symptoms daily or report symptoms you may be experiencing, the following daily screening application described below can be used:

Alfred State College has implemented a new method for completing the daily COVID-19 screening using a SUNY-developed application.

Access the Application

On my.AlfredState.edu you will see the COVID Screening icon listed at the top of the student portal home page. The process for using it on a computer is extremely simple by clicking this icon.

As an alternative, you may choose to download it on a smart phone. Detailed instructions regarding phone apps are listed below.

 

How It Works

When a student indicates “Stayed Home,” Health and Wellness Services will be notified and follow up with the student. Students will receive a copy of any submission via email. Questions regarding this process may be addressed to Health and Wellness Services. Questions or difficulty using or downloading the application may be directed to the Alfred State Help Desk.

If you choose to download on your smart phone

On iPhone

  • Go to App Store
  • Search for “Power Apps” (icon for the app is purple with a rectangle and two diamonds)
  • Download the app and install on your phone
  • Click the icon from your phone screen
  • Login with your ASC network credentials
  • Choose to “allow permissions”
  • Go to “All apps to get started”
  • Choose to “allow permissions”
  • Click “ASC COVID Screening Student”
  • Click the “+” sign to start a new submission
  • Choose a decision (Stayed Home or Attended Class)
  • Save
  • You will then get an email confirmation that you have submitted.

On Android

  • Go to your app store
  • Search for “Power Apps” (icon for the app is purple with a rectangle and two diamonds)
  • Download the app and install on your phone
  • Click the icon from your phone screen
  • Login with your ASC network credentials
  • Go to “All apps to get started”
  • Choose to “allow permissions”
  • Click “ASC COVID Screening Student”
  • Click the “+” sign to start a new submission
  • Choose a decision (Stayed Home or Attended Class)
  • Save
  • You will then get an email confirmation that you have submitted.

If you are experiencing symptoms, please complete the application and stay in your place of residence, Health and Wellness Services will be monitoring these screenings and reaching out any student that reports symptoms with further instructions.
