High School Student Registration Form

The Continuing Education Registration Form [1] must be filled out by all students taking an Alfred State course.

Please be sure to:

  • Fill out all of the data fields
  • Print legibly
  • Have your social security number on the form, it is required
  • Make sure the form is signed
  • Fax form to (607) 587-3295
