Graduate Survey Instructions

  1. Enter Banner Web.
  2. Click Continue.
  3. Click Enter Secure Area.
  4. Enter your SSN as your User ID, then your Banner Web PIN, and click Login. If you forgot your PIN, enter your SSN as your User ID and click the Forgot PIN? button.
  5. Click Student Services & Financial Aid.
  6. Click Student Forms.
  7. Click Graduate Survey and follow the instructions.
